Enclosed are registration and medical history forms for you to complete. It is essential for you to complete ALL of the information on each form. We have standardized forms for both wellness and rehabilitation clients in order to be compliant with HIPAA regulations. We know it takes time to read and fill these forms out and we appreciate your time and attention.
If the forms are not completely filled out upon your arrival, you will be asked to complete them. This will delay and shorten your appointment with your physical therapist.
Please bring the following to your first appointment:
Your prescription to physical therapy. If your insurance plan requires a prescription from your health care provider, it is your responsibility to contact them to obtain a written prescription. The prescription must be completed and provided to Core Therapeutics Physical Therapy prior to or at the time of your appointment.
Your completed paperwork.
Your co-pay.
Your payment in full for the visit (cash-pay clients).
